Camp Waiver

I, individually and as legal representative for the above-named participant, hereby give my permission for my child/ward to participate fully in the CVPWFL 2026 Jim Tully Football & Cheerleading Camp.

I, individually and as legal representative for my child/ward hereby RELEASE AND WAIVE any cause of action and all claims for damages and agree never to pursue any claim, demand, or cause of action against the Connecticut Valley Peewee Football League, its agents, employees, sponsors, supervisors, coaches or assistants for injury arising out of or in connection with my child's/ward's participation in the above named activity.

I, individually and as legal representative for my child/ward, hereby RELEASE AND WAIVE, to the extent not covered by liability insurance, any claims against any person transporting my child/ward to or from activities related to the Connecticut Valley Peewee Football program.
