I, individually and as legal representative for my child/ward hereby RELEASE AND WAIVE any cause of action and all claims for damages and agree never to pursue any claim, demand, or cause of action against the Connecticut Valley Peewee Football League, its agents, employees, sponsors, supervisors, coaches or assistants for injury arising out of or in connection with my child’s/ward’s participation in the above named activity.
